Moultrie County Health Department Temporary Permit

If you are selling any edibles at the CIBR-BBQ you will need to print out and send in this application along with $25 for a temporary permit prior to the BBQ.  If you are selling bottled sauce, you may or may not have to have one, and if they require it you can get one when/if they come around.

Send it direct to Moultrie County as noted on the form.
